Lake County Ohio NAACP Health Committee Mission Statement:
To promote equitable access to healthcare and develop community partnerships to promote the health and well-being of Lake County residents.
Goals:
1) Review data to determine areas of need related to equitable access to healthcare, and develop community partnerships to promote health and well-being.
2) Through advocacy, education, and collaboration with other organizations, community leaders, and stakeholders, effect positive changes in legislation, policies, procedures, and practices that disproportionately impact access to healthcare in historically marginalized communities and those in a middle to high social vulnerability index area.
3) Provide support and resources for those residents needing access to healthcare.
Key
Activities:
1.1 Community Health Needs Assessment (CHA): Assist collaborating partners with primary and secondary data collection for the CHA, by encouraging participation by members, promoting surveys via social media, and assisting in focus groups as needed. 1.2 Community Health Improvement Plan (CHIP): Attend community CHIP meetings and assist with the Access to Care subcommittee as needed. 1.3 Develop community partnerships. Work with stakeholders, community organizations, grassroots organizations, faith based organizations, social service providers, educational providers, community leaders, and health care providers to improve access to healthcare. 1.4 Quality Improvement: Develop a QI Project related to equitable/access to healthcare by December 2024. |
2.1 Advocacy for legislative and policy reform: Engage in targeted advocacy efforts to influence legislation at the local, state, and federal levels. This includes identifying laws, policies, and procedures that disproportionately affect marginalized communities and advocate towards their reform or repeal. Collaborate with lawmakers, community leaders, citizens, policy makers and other organizations to push for changes that promote equitable access to healthcare. 2.2 Community Education and Awareness: Conduct two community awareness/education campaigns per year, utilizing online resources, community partnerships, and media outreach on various topics related to equitable healthcare. Empower community members and collaborating partners to support these educational efforts. 2.3 Foster Community Engagement: Facilitate meaningful engagement between the community and various collaborating partners to promote the health and wellbeing of Lake County residents.
2.4 Determine Educational Needs: Determine through primary and secondary data collection various health care related educational needs by 6/30/2024. |
3.1 Access to resources: Establish an area on the Lake County NAACP Branch website with resources for contacting and scheduling at free clinics, Federally Qualified Health Centers, the local Health Department, nearest Planned Parenthood, and nearest free/low cost dental services.
3.2 Access to educational resources: Establish an area on the Lake County NAACP Branch website for website links to various organizations, such as but not limited to: CDC Vaccine Schedules, ODH website for Children with Medical Handicaps, local Health Department site for car seats and KI distribution, American Red Cross for local blood drives, local FQHC for vaccines |
